The Health Collaborative is a San Antonio nonprofit helping individuals and families improve access to healthcare, health insurance, and community resources throughout Bexar County. They identify barriers affecting people's health and connect them with medical care, housing resources, food assistance, employment services, and other community programs that contribute to a healthier and more stable life.

Their certified Navigators help individuals and families understand their health coverage options, compare plans through the Health Insurance Marketplace, and complete enrollment. Navigators look for eligibility for Medicaid and the Children's Health Insurance Program (CHIP). They connect residents with preventive health screenings, evidence-based care coordination, and other services through community partnerships and neighborhood-based outreach.
