Vision: The Health Collaborative is committed to enhancing positive community health outcomes by leveraging resources effectively and playing a leadership role in evaluating, developing, funding, and implementing health initiatives. We strive to grow our network of partners while reducing duplication of services and promoting coordinated efforts to achieve the best possible outcomes for the community. In addition, we are dedicated to adopting a community health improvement measurement system that ensures accountability and tracks progress toward lasting impact.
The Health Collaborative began in 1997, when the city’s leading healthcare organizations came together to look at the community’s health needs. Instead of competing, they chose to collaborate and share information to better understand the challenges people were facing. In 2000, the group became an official nonprofit with a long-term plan to improve community health. The founding members believed that by working together, they could make a greater impact. Since then, The Health Collaborative has grown into a strong network of residents, community organizations, and businesses. By joining forces, we are able to address critical health issues more effectively, avoid duplicating efforts, and make the best use of resources to improve the well-being of our community.

Our Care Coordination program connects families and individuals with the resources they need to thrive.
Federal Navigator Program
Pathways To Coverage helps individuals and families access affordable health coverage by providing personalized guidance through the health insurance marketplace. Navigators assist with enrollment, answer questions about eligibility and benefits, and connect people to community resources, focusing on underserved and vulnerable populations.
