Community Care Coordination

Our Care Coordination program connects families and individuals with the resources they need to thrive. We support youth facing mental health challenges, people at risk for or living with diabetes, and parents during pregnancy and postpartum. Through navigation support, application assistance, emergency resources, and education, our Care Coordinators make differences in everyday lives.

Federal Navigator Program

Pathways To Coverage helps individuals and families access affordable health coverage by providing personalized guidance through the health insurance marketplace. Navigators assist with enrollment, answer questions about eligibility and benefits, and connect people to community resources, focusing on underserved and vulnerable populations.
