We provide wheelchair ramps to low-income older adults and people with disabilities identified by local healthcare providers. We only accepts referrals from qualified organizations and healthcare professionals. If you need a wheelchair ramp, please do not apply directly.

Ramp Referral Qualifications and Requirements

Referrals that do not meet these requirements will not be processed. Referrals must be submitted by one of the following:

• Social services agency representative
• Community service representative
• Licensed healthcare professional (e.g., doctor, PA, RN, LPN)
• EMS, Fire Rescue, or Police and associated Liaisons

Wait Time:

Due to high referral volumes, construction times may take 3–6 months or longer.
