
CareLink
CareLink is open to Bexar County residents who do not have private or public health insurance. We will establish a monthly payment plan based on total household income and family size. You will be assigned a primary care physician and have access to healthcare at University Health.
IDENTIFICATION
Provide one (1) form of valid identification for each member of the household. Some examples include:
- Driver’s license
- State identification card
- Resident alien card
- Birth certificate (children only)
- U.S. immigration card
- U.S. naturalization, citizenship or other approved federal documents
- Passport
RESIDENCY
Provide proof that you have lived in Bexar County, Texas by one (1) document that contains your name and home address from the list below:
- Household bill (electric, water)
- Lease agreement
- Medicaid/Medicare/CHIP letter
- Attorney General letter
- Food stamp letter
INCOME
Provide proof of total household income for the past 30 days. Examples include:
- Check stubs showing name, date, employer and gross earnings
- Wage verification form signed by employer
- Document verifying sponsor, who must offer his/her source of income, telephone number and address
- Proof of retirement income
- Copies of documents from federal/state agencies
- Texas Workforce Commission (TWC) notice of benefits
- Social Security disability award letter
- Income Tax Return (complete form)
